Part of the issue with PCOS is inherit egg quality issues, in this case I believe there was never a transfer on day 5, one on day 2 and the embryos have not been of th best quality. It would be rare given his semen parameters that it was the cause of the grade of the embryos

When you get a positive HCG and ultimatley a baby it is because if a healthy embryo(s) not becuase of laying down in a certain position or routine, good luck

The issue here is almost certainly an egg quality issue. While it's not unreasonable to try again, the best chance of success at this point would probably be egg donation, but that also has a high price tag.

The advantage that you have is that you have already had 3 healthy pregnancies. It's not unreasonable to transfer 1 given that. If you make good quality embryos then you may have the option to freeze. But using fresh has a higher success rate, so I would wait to see how many embryos and their ...

If you already have children, I would really recommend only 1. Twins are very difficult to raise, and more importantly have a higher incident of preterm birth, and unfurtunatley adverse outcomes from pre term birth.
