I have an appointment at my fertility doctor on Wednesday and there is a possibility I wish to discuss with her, which I have noticed on this site shows more successful BFP's when the embryos are transfered back to the uterus 3 days or more after collection.
I became pregnant on my first two tries at IVF which were both 3 day transfers. I believe traditionally in the UK they transfer after 2 days but in the US up to 5 days???
If anyone can give me advise or opinions on this or offer any stories, I would appreciate it.

I am from the US. ET is done 2 , 3 or 5 days after the retrieval. 5 day embryo is called a blastocyst which has 60-100 cells with the 2 distinct layer, outer which forms the placenta and inner which forms the baby. chances of BFP's with day 5 are very higher compared to others. Why dont all clinics do day 5 transfer?
1) It depends on how many eggs you have because there is a certain amount of risk involved. Not all embryos can get to blast stage. Some embryos can reach upto 8 to 10 cell stage and after that they may stop dividing. To reach to stage 5, genetic activation also plays a role.
2) The culture in which the embroyos grow is also very important. Earl;ier days they did not have good culture to grow the embroys till day 5. Now a days, new cultures have come into market in which the embryos grow and sruvive till day 5. So, chances of forming blast have increased!
3)In US, doctors are selective about choosing patients going to blast stage.They have their own criterion. Like for me, last time only 2 fertilized and so they did not go for blast stage but this time I had 12 eggs (good oone) and out of which 5 made it to blast stage.
I collected 7 eggs this time (gonal f 375) and 4 fertilized (age 39).
With them, I added my last 3 frozen embies (age 34), which luckily all survived the thawing process.
My clinic recommends at tleast 6 embryos to try to get to Blastocyst stage (day 5 or 6).
Of this 7, only 2 actually got to blastocyst stage. One from age 34 and the other from this cycle, age 39.
My clinic gives the success rate for my age (39) of 40% for blastocyst transfer.
It has been a positive for me but yet to know if one or two have taken, if only one, which one will never be known, although they said the age 39 was almost hatching when ET was done.
Not all embryos get to blastocyst tranfer but the chances are higher that it will take it. It could have been was a big reality for me that I would not have had none for transfer.
Blastocyst has many more cells but it also looks different: it has it's own dna by then, looks a bit more like a mass and it is tought going to get at that stage.
They have higher rate with Blastocyst but not all cycle results in a transfer, so if you have only 3 fertilized or so, they say it is better to have them inside you to try to get to blastocyst . But obviously if you have many more fertilized , it can be worth it to see if they get into blasto stage so they can put in the very strong ones as they already made to blasto.
It is usually done for older women but things are changing. I had once a negative FET with was done with assisted hatching.
So this Blastocyst trasfer has obviously been the best option and it worked for me.
By the way, if you go for 2 or 3 day transfer they give you a definite day for ET, but with Blastocyst, you are 'on call' on day 5 and 6 when they call you that they have made it, so go in asap.

FIRST 3 ATTEMPTS I ONLY ENDED UP WITH 1 EGG TO FERTILISE AND ONLY ONE OF THOSE THREE TIMES DID IT FERTILISE AND WAS TRANSFERED ON DAY 2(FAILED)
THIS TIME A MUCH BETTER RESPONSE - HAD SIX EGGS TO BE COLLECTED BUT THEY ONLY GOT 3 FROM ONE OVARY BECAUSE UNFORTUNATELY THEY COULDN'T GET TO THE OTHER OVARY BECAUSE OF BLOOD VESSELS ETC. WAS A BIT DISAPPOINTED 3 WERE LEFT BEHIND BUT WAS ALSO EXCITED TO GET 3 OF WHICH 2 WERE FERTILISED AND TRANSFERED ON DAY 2 AGAIN. (AGAIN FAILED)
I NOW WONDER IF I GET 2 OR 3 FERTILISED EGGS NEXT TIME SHOULD I ASK IF THEY COULD WAIT TO SEE IF ANY OF THEM GET TO BLASTO STAGE BEFORE TRANSFER??
I don't think so Caz.
If you have 2 or three it is better that they try to get to blasto inside you, after all this is the best place to be.
I would ask for assisted hatching if I had only 2 or 3, and put the better looking 2 inside and freeze the other.
The medium which they grow the blasto, i think i read somewhere is not as perfect as inside your womb, but when you have many fertilized eggs, you can afford to see which ones are really strong ones even when you know you will loose some, increasing your chances to putting the best ones and not picking the ones which may have failed .
I don't know your age, but I had my third ivf when i was 36 and it was a 2 day transfer. So it does happens wth 2 day transfer.

I'm at a new clinic for this cycle, and they grow the embies to blatocyst stage for ET (my last clininc did day 2 transfers, 2 FTEs both -ve).
My doctor said that for embies to survive, they need 1. to be strong & 2. to be healthy. The argument is that only the 'strong & healthy' embies grow to the balstocyst stage, therefore more likely to go on to become a healthy, growing baby. But as everyone has said above, the chances of the embie surviving in the lab to the blasto stage is less, whereas it may have survived in your uterus if transferred at day 2.
I think you need PMA whichever way you go (it can't all just be about science!).
