TESE and/or DNA Fragmentation Info

Has anyone's DH done TESE and how did things turn out for you? Our RE is recommending TESE for our next cycle because of DNA fragmentation which may have led to a low fertilization rate for our 1st cycle (and subsequent BFN). Anyone have information on TESE and/or sperm DNA fragmentation? Thanks.

My DH had a SCSA (DNA fragmentation analysis).

Out Dr said a high level of fragmentation could be addressed by antioxidents (cannot remember the specific dose) and/or removing sperm from the testes.

Apparently a great deal of damage to sperm dna can occur in the trip down the urinary tract.

I do have some articles that my hubby downloaded from PUBMED. Basically they refer to studies showing the correlation between sperm fragmentation and low fert and preg rates. They are medical articles so some of the stuff is a bit complicated but they are not too hard to understand.

My DH had a asperation (done with a needle) first which they found a few decent sperm but not good enough to freeze. They did this first to see if a TESA would be worth while. When I had my eggs retrieved (10 of them) they did DH TESA the same day. Sadly they only found three abnormal sperm in a whole tissue. We were very sad as the whole process cost us so much money.

My DH had only one sample out of many in the past year with any sperm, and they weren't moving. He did clomid for almost 6 months and still nothing. They did a TESE and got 26 sperm the day before my ER. Only 3 fertilized and one arrested day 2. We transfered his 2 (5 cell, 8 cell) and an 8 cell from a donor sperm. We decided to go with a donor back-up because my RE was worried about the viability of DH's sperm. It was a tough choice to add the donor embie in but we knew the next stage would be donor and are paying for this out of pocket so we wanted to up our chances.

We just got our BFP today!!

He was pretty sore after TESE for a couple days but it was worth it. Sorry for the decisions you guys have to make. Best of luck.

Samara, if you would send me links to the articles, that would be great. I'm all for being in the know for these types of things. Thanks so much for the info. Are you considering another cycle of IVF? Are they recommending TESE for you? Our RE did say to take vitamin C and E but that the research is less than conclusive on the effects.

Rubylee - Congrats on your BFP, that's so wonderful. I'm sure the donor backup was an incredibly hard decision but it sounds like it's all working out for you guys.

Kery - I'm sorry to hear it didn't work out for you. Do you know what your next move is?

We ended up (after many heart to heart talks) using a donor and got a BFP on the 2nd iui cycle.
