My husband and I have male factor problems (ejaculation problems, low morphology- 8% strict- and a varicocele). motility and count are normal in SA. two questions...

(1) we want to bypass IUI since our chances will be lower due to the low morphology...do you think this is being overzealous? our insurance pays 10k per year for infertility services so we don't want to use up all our insurance coverage of IUIs (doc gave us about a 10% rate per cycle). I know 8% isn't that bad, but I am anxious to get pg and I don't want to go through 4 cycles of IUI only to then start IVF...

(2) What are chances of IVF sucess with low morphology? Is 8% even considered low morphology for IVF? I read it is borderline low for IUI but not sure about IVF. First doc we saw at Columbia said we would need to do ICSI.

You have great chances with IVF due to ICSI. My husband has a 2% morph and they did ICSI to fertilize our eggs. I think IUI with all that gives you a slim to none chance, I would go straight to IVF if I were you.

mb-- my DH has 1% morphology. We did ICSI our first two cycles with BFN. I also have strange eggs with a crater like dent in them so the embryologist thought that the ICSI was harming my eggs with the needle. So the thrid try we did it without ICSI knowing we had a good shot that no eggs would fertilize, but we had 3 do it on their own! That 1% did their job in the petri dish! Anyway, end result is Jack who is 8 weeks! Good luck! I would do IVF though, I wouldn't waste time with IUI, but thats just my opinion.

Neffi- congrats! that is such a great story and I am so happy things worked out for you guys. I think I am going to stick to my guns on the IVF/ICSI...I think we have the best chance and I don't want to waste time on IUI.

Hey there-- you definitely have enough sperm for ICSI to work.
But here's something to consider before moving ahead if you haven't already had it done......
WE had to use icsi due to lower numbers, poor motility and morphology at 2-3%. In 2 fresh cycles I had 40 eggs retrieved. Most fertilized, but only ONE resulted in a live birth. I've had embryos stop at 3-5 days, and chemical pregnancies and miscarriages from the blasts that did make it.

So after all that, the docs sent sperm out for SCSA and we learned that he had lots of sperm defragmentation, which drastically lowers even IVF-ICSI chances of success. I'd suggest SCSA now if you're looking to save time and money now and in the long run.

BTW--Varicocele surgery has improved his motility but they didn't retest morphology nor SCSA. We are going in to ask for that next month....

My husband and I are in a similar situation. He has low count and low motility. Originally we wanted to do IUI because it was cheaper and less evasive. But after speaking with our doctor and others we have decided to do IVF with ICSI. We have decent coverage for fertility and we felt the same as you not wanting to waste money on IUI. Good luck
