AFC question...

Okay - Had my 3d Sono yesterday and my AFC was 9 on my right ovary and 5 on the left ovary. SO, a total of 14. Is this low or normal? I am 34 years old. In 2006, they retrieved 25 follicles and I got OHSS pretty bad. Does it appear, according to my AFC, that I may not get as many follicles? I am so desperate to believe that b/c I REALLY don't want to get OHSS again! I want to actually have a completed transfer from this IVF cycle...

14 is pretty typical. Antral follicle count provides an estimate of how many follicles will develop. Your chance of significant OHSS with 14 antrals is low. However, if many more than 14 actually develop into large follicles, then your risk would increase.

It's also very believable that your OHSS risk would decrease from age 31 to age 34.

Thank you Ghost! I was hoping to hear that! Just got my AMH and it's 2.9. Which my RE said was in the normal range. Our plan is start me on 75 units of Gonal F. (In 2006, it was 150 units of follistim). We will do micro-hcg doses if we need to, but we are playing it by ear for now. My RE is going to put me on a drug, of course I don't know the name of it, after the trigger shot that is supposed to help with OHSS. I think I take it several times... I will have to get the name and see if you know anything about it...

I know that he didn't say the word "cabergoline", but he did say it was a dopamine agonist. Which I'm finding is the same thing.

My big question is, Would this drug affect implantation of the embryos?

Also, does having OHSS affect embryo implantation?

I have not seen anybody report that it does, but it is conceivable that it could. I doubt it has a big effect on implantation, or the studies would have noticed it by now.

It's being used routinely at many centers now. It was first reported by Pellicer's centers, out of Spain. They have huge volume (lots of medical tourism from Italy and Germany due to restrictive regulations there), much of it donor cycles, and avoiding OHSS is a big issue in donors because they are usually young and have high response. Anyway, they know their stuff very well and do excellent research.

It's been shown to reduce the incidence/severity of early onset OHSS (from the hCG trigger) but not late-onset OHSS (from the hCG produced by a pregnancy).

I'll take that! If it will get me thru transfer - I'm a happy duck... If I'm pregnant, I'll deal with the OHSS at that point. I really have a feeling that I will get a mild case of OHSS, but no where near what I had before. My RE is really trying to stay on top of this for me. I hope it all works out!
