Ghost need help_Day 3FSH and Estrogen Level

Hi Ghost,

need your advice, my Day 3 FSH is 7.5 and estrogen was 78. I am 41 years old, is this # good or bad or boarder line?, I just failed my first cycle due to low fertilization rate and only one 4A grading day 3 embroy being transferred.

I have asked another RE for 2nd opinion, he told me that day 3 estrogen level need to be lower than 50 in order to ensure the quality of egg is good. Is that so? I read through the web site, some web site stated as long as estrogen level is below 80 or 90 should be good enough.

I need to find out what cause this cycle's failure in order to improve my next chance. My original RE seems no clue about what went wrong other than we did not do the ICSI due to all sperm results are perfect.

Now, if I have really high estrogen level @ day 3, then how can I improve it?

You might be gong for two different meanings there. If you are talking about a baseline reading of FSH and E2, then E2 above 50 or so may indicate a developing follicle (we call them cysts when we don't want them, follicles when we do want them, but they are all really the same thing). The hypothalamus (a gland in your brain) senses the E2 and releases less GnRH than it otherwise would if E2 was lower, and the reduced input of GnRH results in reduced FSH release from your pituitary (another gland in your brain). In short: High E2 can mean your baseline FSH was optimistically lower than it should have been. In other words, it can mean your FSH of 7.5 is an unrealistic measure of your ovarian reserve.

On the other hand, if you are talking about E2 level at the start of stimulation, then again an elevated E2 level can mean the ovaries are not quiescent, that a follicle is already developing, and that you will not develop a good cohort because the one follicle will dominate.

If it's the former case, then getting the E2 down will allow a realistic measure of FSH. If it's the latter case, then getting the E2 down should allow a better follicular cohort to develop.

Getting the E2 down requires getting rid of the follicle (cyst). This can be done in various ways. You can wait it out, or get rid of it with a little hCG to ovulate it. I suppose a dose of Lupron should do the same (it causes an LH surge with the first dose, and LH or hCG will cause ovulation).
Avoid IVF and surrogacy in Ukraine. Ukrainian centers pay shills to post here under numerous sock accounts pretending to be patients in Ukraine. Centers using such deceptive advertising cannot be trusted and should be avoided.
