Anyone need sperm aspiration for icsi/ivf?

Hi. I'm new to these boards. i was wondeirng if anyone has needed sperm aspiration w/ ivf/icsi. my husband had a vas. some time ago. then last jan we did a reversal. we thought it worked as the first s/a was low but there were viable sperm. after about 6 months nothing and i ma gettting older....38 now. so we saw an re. during the workup we found that he has nothing now. the re is very confident saying that they just aspirate or do some sort of biopsy to get the sperm out. anyone w/ this experience? is it the same day as the retrieval? is it painful??? thanks so much!! we are frustrated!!

Abd sperm aspiration for ivf/icsi- a lot of ladies on these boards have gone thru with it- if you post either the same thing on the general forum or just wait a bit, it'll catch the eye of more ladies..
Me and my husband did not go thru sperm aspi, but just wanted to say hello and welcome you.

Hi Mara and welcome!

My husband had a sperm retrieval (there is two technics called MESA and TESA) .
He also had a vasectomy but we did not try the reversal and went straight for the sperm retrieval.
It was a 20 min operation under GA, a 1.5 cm cut on one of the testis, and the doc got the sperm with a needle.
Normally it is done on the day of the egg collection (then the sperm is fresh!) but it can also be done before and the sperm frozen (as we did).

My husband left the hospital two hours after waking up, he did not have to go back for the stitches as they go on their own when the wound has healed. It was a bit painful and he had to take paracetamol for a few days. Then all was a bit swollen and bruised for a week or so...
That's it!

But I think the worst part for my hubby was when the hairs started to grow back (he had to be shaved!)
So nothing terrible, and as soon as the op is finish you can know if it was sucessful or not.
That why we did not go fo the reversal, I could not wait one year (my doc told me it could take up to one year for the sperm to be back if the reversal is sucessful...)

But you have to be aware that (if i am not mistaken) it is impossible to try another reversal after a sperm retrieval.

Souris: well that is exactly what we need done as i mentioned my husband had a vasectomy. thank you so much for the information. so you did it prior and froze it? my re said he has never heard of a situation where a man who has no sperm in the ejaculate due to a vasectomy or failed reversal was not able to have sperm retrieved. we konw there are sperm in there b/c for a few months he had sperm in the ejaculate...just a very low count. is that similar to what your dr. told you? i only ask b/c you mentioned that they tell you right away if it worked and i was wondering, what happens if you are doing it on egg retrieval day? doesn't it have to work or you havea serious problem? just curious!!thanks for the info...

Hello!

Well my doc was very confident, he told me that sperm retrieval has a nearly 100% success (I guess complications can always occurs...)

I also think it must be a pretty sure procedure as it is normally done on the EC day, and I don't think they would put the ladies through the hassle to have the tmt if there are not many chances to get the sperm from the retrieval!

Sorry if I did not explain myself too well :oops: , what I meant was that compare to reversal, where you have to wait to know if it is a success, the result in sperm retrieval is immediat!

My hubby had his op done prior to mine, because with his job, we couldnt plan in advanced if he was going to be there for the EC (and has it happened, he was not there for the whole tmt,) so the frozen sperm was available at any time!

Thank you so much for your response. it is comforting that it has such a high success rate. and also i am starting bcp's for suppression b4 my dh has his urology consult so they must be confident!! are you in the middle of a cycle now? that is great that they froze them . my re would prefer to do ti the same day, if possible, so we are going to make it work. but i wouldn't mind having frozen as a backup since you never know what's going to happen.

Hi Mara!

I think in any cases, if they retrieve enough sperm for more than one ICSI (and they will, because they only need one sperm for each egg they want to fertilise) they will freeze the rest... for future cycles if the first one fails (sorry to say that...) or for future babies!

Unfortunately I am not doing any tmt at the moment, it is planned for mid January... I cant wait! My first ICSI was in April, but it did not work... And for financial reason we have to wait all this time for the 2nd try :evil:

I really feel it is going to work this time (it was a real shock for the first time as I thought it was going to be a BFP!)
My doc was so confident, because I am 23 with no fertility problems (well we dont think so, as i have never try to get pregnant before... so who knows?) and the only reason we could not conceive was because my DH had a vasectomy.
But out of 11 eggs, only 3 fertelised (DH sperm was not as good as we thought!) and the 3 embies were transfered but none survived...)
And this time, my new clinic will transfer only 2 embies...

thanks for your help!! im sorry your 1st icsi didn't work. im afraid im headed down that road as well and we are out of pocket as well so we will need to take some time in between as well.

do they know why now that it didn't work? was it the sperm quality? i was under the impression that that didn't matter too much but maybe it does. how long has your husband had his vasectomy for ? does he have children? this whole process is quite annoying!!

Hi Mara!

Dh had his vasectomy done (i think ) about 8/9 years ago... and yes , he has children (and grand children...).
It is difficult sometimes, because he knows how much it means to me to get pregnant, but obviously, he has already children... So if things doesnt work out, I think it will be a lot harder for me...

Well, out of 11 eggies, 3 fertilised . So I was happy to have 3 embies, but the doc was expecting a lot more... and this was due to Dh sperm...

But none of the 3 embies transfered made it...The doc had no explanation for that... I guess it is just the nature (easy to say but hard to accept!)

DO you know when you will start your next tmt?

well, as soon as af shows (the end of this week) i am supposed to go on bcp's to suppress and i go from there.......lupron.....and then stims and you know the rest. my dh has his eval w/ the urologist in about a week or so but the Re has scheduled us to start my suppression month figuring that dh has the sperm but it's just not coming out...

so we'll see. i am concerned about the viability of the sperm...the re said that isn't so much an issue w/ the icsi but i still keep feeling negative. do you know anything about that? viability of sperm and whether it matters w/ icsi?

we'll see. i gotta run but will write more later......... hope you are well.

Hi Mara,

I am also quite new to the board. I visit this site from time to time to catch up with the news.

My husband also had a vas. years ago. Like Souris and her husband we decided to go straight to ivf/icsi by sperm retrieval. The operation procedure is the same as Souris described, and it took about 25 minutes. Two hours after the operation he was allowed to go home. There was some bruise and swollenness, but decreased after a few days. The doctor took out sperms just enough for 2 cycles. Now we used all the sperms and unfortunately we haven’t got our luck yet. If we want to have another go at ivf/isci cycle then my husband will have to go through another sperm retrieval again, but we think it will be worth going through at the end.
