Ghost - PIO Injections

Post by ktelise »

Ghost - Last night when my husband gave me my progesterone injection quite a bit of blood came out after the needle was removed. And this morning I have a large knot the size of my hand on my rear and my muscle is sore going down my leg. He did pull back the needle before injecting and it was fine. Can this be harmful to me or the baby?

What could have happened? Any recommendations?

Sounds like you hit a small blood vessel. It happens. You may have lost some of the injection through the immediate bleeding. Let your clinic know. They may have some instructions for you, such as an additional injection (excessive progesterone is not a problem), depending on where you are in your cycle/pregnancy. Many patients have way more progesterone than they need at certain points.

Don't worry over it.

Ghost - Thank you so much for the quick information! I am also taking 200mg of progesterone 3 times a day and my last progesterone draw was 48. I do have a call into my clinic to let them know.

Can you tell me what my progresterone level should be at this point? I will be 6 weeks tomorrow.

Thank you!!!

The protocols I have seen have minimum progesterone levels of 10, 15, or 20 ng/ml. These have safety margin built in. I've read the critical level not to go below is actually around 6.5.

I've never heard of an upper limit, and I know of cases with over 400 pg/ml measured days after retrieval.

Hi Ghost. Can you tell me when it is safe to stop taking progesterone in early pregnancy. My RE wants to start to reduce my dosage and I'm only 7w3d. A couple nights ago when I first reduced my dosage in half I work up to pee that night to find brown discharge with AF like cramps. I hadn't had any cramping before and the only thing that changed was my reduction in PIO dosage. Can this happen when you reduce your dosage level? My progesterone level was 40 and my RE said he didn't think the cramps/discharge was from the reduced dosage. He thought it was from my utereus stretching/growing (esp given that I am carrying twins). I had an u/s the previous day and the heart beats were strong (about 130 each), size was good and my hormone levels are good. And he said if it is a m/c there is nothing he can do to stop it given all my numbers and such look good.

After much discussion my RE said I could go back to my original dosage and since doing so the AF like cramps have gone away. RE said I need to be weaned off before I can be released in one week. I didn't think my body/the placentas were making enough progesterone on it's own yet to sustain the pregnancy (thus the purpose of supplementation). Can you give me some insight to why they are reducing my progesterone dosage so early (besides so that they can release me)? Most of my fellow IVF pregnant sisters on this board are on progesterone of some kind until 10-12 weeks.

We keep them on progesterone to about 9 or 10 weeks. We check their levels to make sure they are high enough (it starts rising around that time because of the placenta making progesterone).

I know of no general rule that a patient must be off progesterone when their care is released to an ob/gyn. We've had the rare patient who was still on progesterone when we released them. Ob's prescribe progesterone in some cases.

Your level of 40 exceeds every protocol I've seen (they specified 10, 15, or 20, and there's some safety margin built in to those minimums). I suspect many doctors would have cut your dosage with a level of 40.
