I am unable to get a vial of the same donor's sperm, while I have 5 day 5/6 embies using his DNA vitrified. At my last retrieval I asked the embryologist to refreeze any leftover sperm, which he did.
I will once again be using ICSI for my next banking cycle and would like to try using the rethawed sperm. My embryologist has never had to deal with a problem like this in all his years of experience, so he cannot tell me if if will work or not. I will probably have a new donor's sperm as a backup, but that won't be my first choice.
Has your clinic had to use twice frozen sperm again and what was the outcome?
I have not forgotten this. It's on a sticky note so I will remember to ask the docs if/when I can get a chance.

I asked. We've tried re-freeze/re-thaw once or twice with sperm from cancer patients, who could no longer produce any more. Nobody recalled the outcome of those cycles. It's unusual, and it's been awhile.
