We were supposed to have our day 5 transfer today, but received a call that it was being pushed back. Just wondering if a day 6 transfer is better than a day 5? I know that a day 5 is better than a day 3 due to being able to pick the stronger embryos. Common sense, would say that a day is better, but don't you want to get the buns in the oven already?
Day 6 is inferior to day 5. A day 6 transfer USUALLY means your embryos did not blastulate on day 5, so they are waiting to day 6 to get good blastocysts. If it was delayed just for convenient scheduling, then that's entirely different from what I'm talking about here.
Day 5 blastocysts have much better implantation rates than day 6.
Typical good day 6 transfers have about one chance in three of resulting in live birth. Day 5 gives about one chance in two. However, day 6 can be virtually as good as day 5 under certain conditions (low progesterone on day of hCG, slowing endometrial development).
The problem with day 6 is not bad embryos, but that they are more likely to miss the implantation window. There is a window of 24-48 hours for the embryos to implant, and slow embryos can miss the window. That's more likely to occur for day 6 than day 5 blasts. However, day 6 embryos do just as good as day 5 in frozen cycles, where they get an equal chance of meeting a medically controlled implantation window.
Transferring them earlier will not help. They can't implant until after hatching, which occurs about a day after blastulation. The issue is slow embryos, not late transfer.
By the way, most clinicians are not aware of the above, or are only vaguely aware. It can be pieced together from many publications, but most clinicians think it's all about the embryos and all but ignore the endometrium as a source of failure. Yet the day 5/6 effect is all about the endometrium.
My personal advice is to be very open to the option of frozen transfer, if your clinic is good at it.

All embryos implant at about the same time, regardless of when transferred. That is, around day 6 or 7. They must first grow to blastocysts, then hatch (shed the zona). Transfers on day 3 will not implant for 3 or 4 days. Transfers on day 5 or 6 might implant the following day or two. This explains why clinics that do day 3 ET tend to wait a few days longer, measured from the transfer point, for their pregnancy tests. If measured from blastulation or implantation time, they are all about the same.

hi....having read over these posts i noticed the information given is really good.....
i just want to say that i had a 3 day transfer,which i had to test 14 days later...though i had symptoms of pregnancy from day 8 onwards....i am pregnant....
i feel that to say a 5 day transfer is better than a 3 day is a little hurtful and may lead other women to feel slightly down.... 5 day transfer mabye slightly better as to which points out which embryo(s) look better but there is no accuracy to say that a 5 day transfer will work better than that of a 3 day!!!! further more not all clinics go for 5 day transfer....
it all depends on the embryo being placed back in the womb if its accepted or not....2days...3days or even 5day transfers....lets just hope and pray either work....

Wanted to thank you again for the very informative information, i wanted to know if you can tell us a bit about Immunolgy testing regarding infertility.
little bit of info on me, both my husband and i are in good health and we produce great embies.. this last cycle we had all but one compacted past 16cell on day 3. im currently on my 2ww and testing on monday.. We had wonderful embies on our last shot as well unfortunately i ended up without a pregnancy. This time around he told me that women who have this good quality embies 90% of the time get pregnant, and that if i didnt get a positive i need to consider a surrogate? Does this make any sense for someone who has no problems and husband has no problems as well.
I wanted to know if have immune testing done would some how resolve any of my issues, because i am soooooo scared that this cycle hasnt worked either i did do a hpt today and it was negative, and i am truly devastated and scared.

ghost a qucik question ..when reviewing fertilized eggs for feezing can i ask please what would be a criteria difference for fresh and freezing..i mean if a woman where to have 20 eggs removed 10 fertilized and 2 where chosen for transfer what would cause the others to not qualify for freezing ?? i am curious because as you know (god Bless you) i had the 2 transfered but there where none left that where viable to freeze?---thanks kell
Thanks Ghost. I am trying to stay posiitve. Out of 16 embryos I had 4 that made it to morula on day 5. The clinic made me wait until day 6. I am so worried about the low odds. The RE said my day 6 blast was very good. Does that change my odds?
Since I last posted, we were actually pushed to a day 6. On day 6, two of the embryos were morulas and one was a full blastocyst. They out in all three. Our bloodtest is this coming thursday. However my wife has been experiencing sore, tender breasts, blue veins in her breasts and has had some spotting and cramping. Are we looking too much into this or could this be signs of pregnancy?

I can say a fully developed expanded blastocyst on day 6 is certainly a good sign.
