I also want to let you know that I did get pregnant once naturally in 2004 and miscarried after 8 weeks. Tried for a year naturally with no success and then went for infertility treatment.

Two factors were working against you. The one everybody talks about, embryo quality, was not in your favor. The rapid failure of 43 oocytes/embryos did not bode well for the remaining three. The other factor, the one few talk about, is endometrial receptivity, which is frequently compromised in cycles with many follicles, and you must have had around 50 or more follicles in order to get 46 oocytes.
Most IVF cycles fail, and yours was probably compromised anyway, but few with so many eggs have so few viable for transfer/freezing. That's why I am more curious about that aspect.
Were you coasted? Triggered early? Given a Lupron "trigger" or perhaps a reduced dose of hCG? Most clinics get nervous with patients with 50 follicles, because of the risk of OHSS. They might compromise the stimulation and therefore the oocytes.
