The 'ideal' size of follicles varies by patient, and different centres have different criteria for how follicles are measured...so there is no perfect score.
I have seen patients achieve pregnancy from follicles as small as 14mm and as large as 27mm.
That said, of course, we try to optimize cycles by triggering ovulation at the best possible moment.
During my training, a decade ago now (!), I was taught that the ideal follicle sizes were 20 to 23mm. More recently, some European centres are suggesting that the lead (largest) follicle be no more than 17mm on the day of the HCG trigger shot.
I believe the truth is somewhere in between, and am happiest when the bulk of follicles are 16mm or larger.
(Yes, they continue to grow).
