I did a 3dt my 1st IVF cycle in April...I am now 27 weeks with a baby girl! My RE felt embies do better back in the uterus, although I've read a 5dt is closer to when a natural PG would implant.
It's God's call if the embie implants or not...so whether you do a 3dt or 5dt if it's meant to be it will be.

hey Gina, this is my take on things...

Not all 3 day embies can carry on growing, in fact only 60% make it to 5days, so if you have a 5 day embie you know it is definate a strong and viable embryo. SOme people say that the body is a better environment, however, so some embryos that do not make it to 5 day in the lab may have made it to 5 days inside the uterus as its a natural environment.

I know that putting 3 x 5 day embies (blasts) is more likely to result in multiples than 3 x 3 day embryos for that very reason.

In an ideal world i would like to put in 2 blasts but as i produce very few embryos, i have to be satisfied with 3 x 3 day embryos. If i can produce more this time (i am on DHEA and a different protocol) i will elect to do a 5 day, although my RE says he favours 3 day transfers. All RE's have their own take on this, however.

Hey Gina: here's what we did. Our first IVF we had only 1 embryo to transfer which was a 7 cell. We transfered on day 3 and got a BFN.
Our next IVF we did 3 day 5 blast and got pregnant with a singleton but lost the baby at 11 weeks.
This time around we transfered 3 day 5 blasts again and we're currently pregnant with boy/girl twins.
Now, just so you know, the reason we did things that way where as follows, they tranferred the 1 enbryo we had on day 3 because they where afraid of the only enbryo we had not making it to day 5 not because it was a good quility embryo, but because it was all we had and they didn't want to take a chance of anything happening to it. The uterus is the best place for an embryo, not a petrie dish.
the next 2 times my husband and I where the agressors against our RE recommendations because we where afraid of NOT egtting pregnant. Our embryos have been perfect or near perfect but our fear of not having the funds to do this again and again made us be more agressive with good quaility embryos.
There's a lot of reasons why to choose a 3 day or a 5 day. As you can see our decision had nothing to do with quailty but more with desperation.
all in all it really depends on how your babies do and what chance your willing to take with them if they're great looking.
Good luck in anything you choose and please keep us posted.

Hi Gina,

From what I have heard from my RE - I don't know if it is truly our decision to make on the 3d or 5d. I can tell you that ordinarily 5d is preferred.. but, a lot of embryo's aren't strong enough to live outside the uterus for 5 days.

From what I have heard, if you have strong embryo's & a decent amount of them, by holding them out until blast stage, they can identify the stronger embryo's... but, on a day 3, if you have a smaller amount of embryo's (even if they are strong), the doctors might not want to risk holding the embryo's outside the body..

I was originally scheduled for a day 3 transfer.. and they called me this morning (for what i thought was the time to come in) and they told me that they wanted to push to day 5.. so, i am going in on Friday instead.
I had 6 strong embryo's on Monday - although, i am unsure if that number has since dwindled... I guess I will learn everything on Friday.

Best of luck to you and I will be spreading baby dust all around!!

I prefer day 5, if there are many embryos to choose from. Day 5 are easier to grade, and 5-day culture screens out those that just can't get that far.

But most clinics charge extra for 5-day culture. Also, if it's a smaller cohort, it will usually be pretty obvious which of them are the best on day 3. No need to spend the extra money in that case.

I did 5 day transfer. My RE put 3 5 day blasts back in me. For financial reasons we decided 3 instead of 2 because of my age and my RE agreed that was a good idea. We also had decent qulaity blasts but not great. Everthing I read, along with my discussions with my RE told me that multiples where less lkely on woman over 35. Well in my case all three took. At 7 weeks we had three heartbeats, at 9 weeks we had two...I miscarried baby C but the twins are ok. I know allot of woman who put in 3 5 day blasts and ended up with one baby. I also have read allot of woman who had twins and some who ended up with a nn viable pregnancy.
I think its a personal decision. I wieghed the risks and decided the odds were low of me having more than 2.....I Know my story is not the norm..but I stillfel good abot our decision

I hope that helps, I wish you the best!!!

Just my opinion, but I would not transfer 3 blasts unless the blasts were very poor quality.

1 or 2 is good in most cases. Most patients like the idea of twins, and the transfer of two good blasts will give a lot of twins.

Ghost, I would never disagree with you normally...but on this I strongly do and feel the need to outwardly disagree.
I think the amount depends on your age, reasons for infertility, and your financial posistion. I think the broad brush stoke of 1-2 is kinda flimsy for woman over 35.

Like I said, it's just my opinion. There is no right or wrong here. We get different opinions from different perspectives and backgrounds. Keep in mind my clinic has a very good freezing program, so there is little or no pressure to get them all in on the fresh cycle. The next transfer is just as good as this one.

The latest ASRM guidance statement is 1 blast for good prognosis, 2 blasts for poor prognosis. An older patient can justify "poor prognosis" based on age, but IMO that is another "broad brush" because some older patients produce great blasts. IMO, it should be based on the blast grade.

Still, if the patient does not mind twins, or even prefers them, I could see transferring two even with good prognosis. Three? No, not unless they were very poor.

I come to respect your opinion greatly, and mean no disrespect to disagree once again, but your last comment leaves me to rebut once again..I have been on this thread a while now and have monitered other forums as well for quite some time...I realize your a professional and I am not ........but I have come to know allot of woman, epseiclaly those over 35 an some younger with various issues, all who transferred 3 5 days blasts and most have come up with one baby or twins...more often one baby and sadly sometimes none. These woman from what they said had decent quality blasts. I think there are to many persoanl variables that help make a decision of how many to put in.....I know that 1-2 good qulaity blasts is the norm an least risky for multiples..I would never advise more than three....but sometimes the norm doesnt apply to special cases......and I dont know all of Ginas info.......and in my case and special curumstances 3 made sense .....So I think Gina needs to weight her own situation against the statistics intead of purely relying on them.

It's no problem. It's not really even a disagreement.

In any case, I posted an answer to your question several posts up.
